Premiums are partially paid by NextStage Clinical. Choice of PPO or High Deductible plan with HSA
Dental
Premiums are 100% company-paid. Choice of providers, with many in-network options.
Vision
Premiums are 100% company-paid. Large network of providers for eye exams, lenses, frames, and contacts.
Life, STD & LTD
No cost to employees. 1x base salary for Life Insurance, and 60% of base salary for STD and LTD

Company matches 100% of first 3%, 50% on next 2%
